Edwin H. Mookini Library Request for Library Patron Information

Policy
The Mookini Library expects periodic requests for library patron information. All requests for Library Patron Information must be in writing, using the Edwin H. Mookini Library Request for Library Patron Information Form (.pdf)

Requests will be reviewed and responded to by the Access Services Librarian. When the Access Services Librarian is not available, the University Librarian, or designate, will be responsible for reviewing and responding to all written requests.

Under normal circumstances, a response concerning a request for Library Patron Information will be provided within two (2) working days.

A copy of the Request for Library Patron Information, along with the Library's response / copy of the information provided, will be maintained in the Library Administration Office for a period of three (3) years.

Library Foundational Principles
The Mookini Library supports and complies with the American Library Association's Policy on Confidentiality of Library Records and the American Library Association's Code of Ethics.
Ethical dilemmas and differences of opinion may occur when information needs, including Campus requests for information, are reviewed by the Mookini Library, in conjunction with foundational library principles of privacy and confidentiality. We ask that requestors respect our need to fairly evaluate each request for Library Patron Information.

Family Educational Rights and Privacy Act
The Family Educational Rights and Privacy Act (FERPA) (20 U.S.C. § 1232g; 34 CFR Part 99) is a Federal law that protects the privacy of student education records. The law applies to all schools that
"Under FERPA, schools may disclose, without consent, "directory" information such as a student's name, address, telephone number, date and place of birth, honors and awards, and dates of attendance. However, schools must tell parents and eligible students about directory information and allow parents and eligible students a reasonable amount of time to request that the school not disclose directory information about them. Schools must notify parents and eligible students annually of their rights under FERPA. The actual means of notification (special letter, inclusion in a PTA bulletin, student handbook, or newspaper article) is left to the discretion of each school."
